MOTHERS DAY BREAKFAST PERMISSION FORM

Dear Families,

DEPS Mother's Day Breakfast will be held this Thursday, 8th May from 7.45-8.45am.

IF YOU ARE BOOKED INTO DEBASCA ON THURSDAY MORNING, please complete the form below, giving permission for your child to participate in the event. DEBASCA staff will supervise our children at a ratio of 1:15.

For allergy management reasons, we ask that any children with food intolerances/allergy to please only eat food from the breakfast under the supervision of their parents/carers.

DEBASCA will still have breakfast available as per usual inside the centre, for those signed into care that morning.

If you are staying for a short while for the event, please ensure you sign your child in when you leave so that staff can resume duty of care once you leave.

All children signed into DEBASCA will be required to wear a YELLOW vest for ease of identification (as per normal). This way, we can more easily collect all students at the end of the event, to mark the roll and dismiss the students.
