Dear Families,
Please be mindful to complete the required permissions using the online form link for 2025 Extra Curricular classes held on school grounds. This includes morning and afternoon extra-curricular activities.
Authorised Person - Extra Curricular
To give permission for an Authorised Person to Collect/Drop Off your child for an Extra Curricular activity that is held outside of DEBASCA's program.
E.g: Music lesson, drama lesson, art lesson, language lesson, dance lessons etc run by external providers on school grounds.
This person will be responsible for signing your child out of DEBASCA for the activity, and then returning your child and signing them back in to DEBASCA.
It may be the activity teacher, or another parent.
Please ensure you discuss with the Extra Curricular provider/teacher to organise this before completing this form.
